Admission Criteria

Qualifiers for Admission

• Age 62 or older
• Ability to ambulate (self-ambulate in wheelchair)
• Requires partial and/or no assistance with the bathroom
• Self-managed portable oxygen equipment with sporadic need
• Early- to Mid-stage dementia
• Mild cognitive impairment
• Needs assistance with up to three activities of daily living (bathing, dressing, undressing, toileting, laundry, medications)
• Ability to follow cueing
• Short-term nursing need
• Needs to pass admission criteria above and face-to-face evaluation within 30 days prior to admission
  
  

Disqualifiers for Admission

• Danger to self or others
• Unmanageable urinary or bowel incontinence
• Unable to self-transfer
• High-risk for falls
• Abuse of drugs or alcohol
• Personal care needs exceed supervision level and simple assistance and/or cannot be met by the staff
• Requires more than a weekly visit from nurses
• Chronically bed fast
• Chronically chair fast
• Mid- to Late-stage dementia
